Leveraging Assets to Create Seamless Services

With experience in more than 200 communities throughout the United States, Tripp Umbach used its breadth of perspective to complete a broad-based community assessment that included program planning and evaluation for Healthy Northeast, a not-for-profit partnership of communities and higher education institutions.  The work provided a baseline to evaluate the unique community-campus partnership designed to create healthier communities through collaborative efforts that built upon the strengths of each organization and leveraged the resources of its partners.  The research helped to build momentum, energize partners and create awareness about the role of community assessments in collaborative health planning and delivery.  Follow up studies have indicated that Healthy Northeast’s model, involving health providers, community groups, schools and community organizations is working to overcome complex societal problems.
