Community Health Priorities for a Region in Transition
The Challenge
In the summer of 2024, St. Clair Health launched a Community Health Needs Assessment (CHNA) to identify and understand the evolving healthcare demands of the region. The goal: evaluate how effectively services meet community needs and uncover areas for strategic focus.
How We Helped
Tripp Umbach partnered with St. Clair Health to carry out a broad community-wide assessment. The process included surveys, focus groups, stakeholder interviews, and a prioritization session — engaging local government, human service agencies, educational institutions, and healthcare providers.
The CHNA identified key priorities including access to care, behavioral health, chronic diseases and cancers, and social determinants of health such as housing and transportation. With these insights, St. Clair Health is aligning programs, partnerships, and resources to reduce health disparities and promote long-term wellness across the community.
