The Challenge
Leonard J. Chabert Medical Center (CMC), located in Houma, Louisiana, has been a cornerstone of care for underserved and vulnerable populations since 1978. Committed to improving the health and wellness of Southeast Louisiana, CMC sought to better understand evolving community health needs and identify actionable strategies to improve outcomes through its 2023 Community Health Needs Assessment (CHNA).
How We Helped
Tripp Umbach conducted a comprehensive CHNA process to assess the region’s health challenges and opportunities. The engagement combined secondary data analysis with extensive community input through surveys, stakeholder interviews, and leadership collaboration. This inclusive approach ensured representation from healthcare leaders, policymakers, educators, social service providers, and local organizations.
Tripp Umbach facilitated a prioritization process that identified three key areas of focus for CMC: chronic diseases, health promotion and education, and access to care. Recommendations included expanding communication and community engagement, improving health literacy, and increasing awareness of available programs and services.
With these priorities in place, Chabert Medical Center is continuing to strengthen partnerships with local institutions and organizations, ensuring its commitment to improving health outcomes, enhancing access, and fostering long-term community wellness throughout Southeast Louisiana.
