The Challenge

Lake Charles Memorial Health System (Lake Charles Memorial) sought to better understand the evolving health needs of residents throughout Southwest Louisiana and develop a coordinated strategy to address the region's most pressing health challenges. Following a comprehensive Community Health Needs Assessment (CHNA), the health system recognized the importance of moving beyond identifying community health priorities to implementing sustainable, evidence-based initiatives that would improve health outcomes and strengthen community well-being. To maximize the impact of its community benefit investments, Lake Charles Memorial sought a strategic implementation plan that aligned organizational resources, engaged community partners, and established measurable goals for improving population health across its service area.

How We Helped

Tripp Umbach partnered with Lake Charles Memorial Health System to conduct a comprehensive CHNA and develop an Implementation Strategy Plan (ISP). The CHNA employed a robust mixed-methodology approach that included extensive secondary data analysis, community surveys, stakeholder interviews, and a community health prioritization process. The assessment evaluated population demographics, health outcomes, healthcare access and utilization, chronic disease prevalence, behavioral health needs, maternal and child health indicators, health disparities, and the social determinants of health affecting residents throughout Southwest Louisiana.

Building upon the assessment findings, Tripp Umbach facilitated a collaborative implementation planning process involving health system leadership, physicians, public health agencies, community organizations, educational institutions, local government representatives, and other key stakeholders. Together, the team identified evidence-based strategies, established measurable objectives and performance metrics, developed implementation timelines, defined organizational responsibilities, and created an evaluation framework to monitor progress over time. The implementation strategy focused on improving access to care, expanding behavioral health services, addressing chronic disease prevention and management, advancing healthcare access, and strengthening partnerships to address the underlying social and economic factors influencing community health.

The Impact

The CHNA and ISP provided Lake Charles Memorial with a comprehensive, data-driven framework for improving the health of the communities it serves. The project identified clear community health priorities, established actionable strategies, and created a roadmap for translating assessment findings into measurable improvements in population health. By aligning community benefit investments with documented community needs, the health system is better positioned to improve access to healthcare services, reduce health disparities, strengthen chronic disease prevention efforts, expand behavioral health resources, and address the social determinants of health through collaborative partnerships.

The implementation strategy also strengthened collaboration among healthcare providers, public health agencies, nonprofit organizations, educational institutions, and community leaders by fostering a shared vision to improve health across Southwest Louisiana. With defined goals, measurable outcomes, and an ongoing evaluation framework, Lake Charles Memorial now has a sustainable approach for monitoring progress, adapting initiatives to evolving community needs, and demonstrating the impact of its community health investments. The project reinforces the health system's commitment to serving as a trusted community partner and advancing healthier, more resilient communities throughout the region.
