Developing a Strategic Roadmap to Address Community Health Priorities and Improve Outcomes Across Southeast Louisiana.
The Challenge
Leonard J. Chabert Medical Center (CMC) sought to better understand the evolving health needs of residents throughout Terrebonne Parish and Southeast Louisiana while developing a strategic framework to address the region’s most pressing health challenges. As a major regional healthcare provider and teaching hospital, CMC recognized the importance of identifying community health priorities, evaluating barriers to care, and understanding the social, economic, and environmental factors that influence health outcomes. The medical center also sought to translate assessment findings into actionable strategies that would strengthen community partnerships, align resources, and improve population health over the long term.
How We Helped
Tripp Umbach partnered with CMC to conduct a comprehensive Community Health Needs Assessment (CHNA) and develop a corresponding Implementation Strategy Plan (ISP). The CHNA utilized a mixed-methodology approach that combined extensive secondary data analysis, community surveys, stakeholder interviews, and a formal prioritization process. Input was gathered from healthcare providers, public health agencies, educational institutions, nonprofit organizations, community leaders, and residents to ensure that the assessment reflected the diverse needs and perspectives of the communities served by the medical center.
The assessment examined healthcare access and utilization, chronic disease prevalence, behavioral health needs, maternal and child health indicators, workforce challenges, health disparities, and key social determinants of health, including transportation, housing, food access, education, and economic stability. Through this process, Tripp Umbach helped CMC identify the most significant health issues affecting the region and establish priorities for future action.
Building upon the CHNA findings, Tripp Umbach facilitated a comprehensive implementation planning process that translated identified priorities into measurable goals, action plans, timelines, and performance indicators. Working collaboratively with hospital leadership, the team developed strategies to improve access to care, expand behavioral health resources, strengthen chronic disease prevention and management efforts, promote health education, and address health disparities across the service area.
The combined CHNA and ISP provided CMC with a data-driven roadmap to improve community health and strengthen its role as a regional leader in population health improvement. The process identified key priorities related to access to care, behavioral health services, chronic disease prevention and management, health promotion and education, and health equity. It also highlighted opportunities for greater collaboration among healthcare providers, public agencies, educational institutions, nonprofit organizations, and community leaders to address the underlying social and economic factors that influence health outcomes.
As a result, CMC is better positioned to align community benefit investments, organizational resources, and partnership initiatives around shared health goals. The project established a framework for accountability, performance measurement, and continuous improvement, enabling the medical center and its partners to track progress and adapt strategies as community needs evolve. Ultimately, the CHNA and ISP reinforced CMC’s commitment to improving health outcomes, reducing disparities, expanding access to services, and advancing the overall health and well-being of communities throughout Terrebonne Parish and Southeast Louisiana.
