Penn Highlands Healthcare

Advancing Community Health Through Data-Driven Collaboration

Three generations of a family laughing together as they embrace.

The Challenge

With nine hospitals spanning Northwestern, Southwest, and Central Pennsylvania, Penn Highlands Healthcare is deeply committed to improving the health and well-being of the communities it serves. In early 2024, the health system launched a comprehensive Community Health Needs Assessment (CHNA) to better understand regional health priorities, disparities, and factors influencing outcomes. The goal: to use data and collaboration to guide strategies that deliver measurable improvements in care, equity, and access.

How We Helped

Tripp Umbach partnered with Penn Highlands Healthcare to lead the CHNA process—a systematic evaluation designed to identify and assess community health needs. Through extensive data collection, stakeholder interviews, and regional analysis, the study provided a detailed picture of current health conditions and the social determinants shaping them. The assessment equips Penn Highlands with actionable insights to inform strategic planning, resource allocation, and community partnership development.

The Impact

The CHNA findings position Penn Highlands Healthcare to leverage its extensive network, resources, and relationships to address the region’s most pressing health challenges. Working alongside local partners, the system is focused on implementing targeted initiatives that enhance health outcomes, promote wellness, and strengthen access to care across its service areas. These collaborative efforts underscore Penn Highlands’ ongoing commitment to creating healthier, more resilient communities across Pennsylvania.

Every successful project starts with a conversation.
