Identifying Community Needs and Developing Strategies to Advance Population Health

The Challenge

Terrebonne General Health System (Terrebonne General) sought to better understand the evolving health needs of residents across Terrebonne Parish and develop a strategic roadmap to improve community health outcomes. As the region's leading healthcare provider, Terrebonne General recognized the importance of identifying the most pressing health challenges, understanding barriers to care, and evaluating the social, economic, and environmental factors that influence health and well-being. Beyond assessing community needs, the health system sought a structured implementation strategy that would translate findings into actionable initiatives, measurable outcomes, and long-term community impact.

How We Helped

Tripp Umbach partnered with Terrebonne General to conduct a comprehensive Community Health Needs Assessment (CHNA) and develop a corresponding Implementation Strategy Plan (ISP). The CHNA utilized a robust mixed-methodology approach that included extensive secondary data analysis, community surveys, stakeholder interviews, and a formal community prioritization process. Input was gathered from healthcare providers, public health officials, educators, nonprofit organizations, social service agencies, community leaders, and residents to ensure broad community representation.

The assessment examined population demographics, health outcomes, healthcare access and utilization, chronic disease prevalence, behavioral health needs, maternal and child health indicators, health disparities, and key social determinants of health, including housing, transportation, food security, education, and economic stability. Through this process, Tripp Umbach helped Terrebonne General identify priority health needs, emerging trends, and opportunities to improve health outcomes throughout Southeast Louisiana.

Building upon the CHNA findings, Tripp Umbach facilitated a comprehensive implementation planning process that translated identified priorities into actionable strategies. Working closely with health system leadership and community stakeholders, the team developed measurable goals, implementation timelines, performance metrics, partnership strategies, and accountability structures. The resulting plan focused on improving access to care, addressing chronic disease, advancing health promotion and education, and strengthening collaboration among healthcare, public health, education, and community organizations.

The Impact

The combined CHNA and ISP provided Terrebonne General with a data-driven roadmap to improve community health and guide strategic decision-making. The process identified key priorities related to healthcare access, chronic disease prevention and management, health education, and promotion. It also established a clear framework for aligning community benefit investments, organizational resources, and community partnerships around shared health goals.

As a result, Terrebonne General is better positioned to address healthcare disparities, expand access to services, strengthen preventive health initiatives, and improve population health outcomes across Southeast Louisiana. The project reinforced Terrebonne General's role as a regional health leader and created a sustainable framework for ongoing evaluation, community engagement, and continuous improvement in the health and well-being of the communities it serves.
